We are seeking a talented Violence Prevention Case Manager to join our team of nearly 500 employees. CCSI is contracted by Rise Up Rochester (RUR) to provide program management services, including specialized staffing. Rise up Rochester is a nonprofit that empowers the community to establish and maintain a nonviolent culture. Rise Up Rochester provides assistance to individuals affected by crimes and their families.

The role of the Violence Prevention Case Manager provides intensive, trauma‑informed case management services to individuals directly impacted by violence. This role supports clients through safety planning, stabilization, housing navigation, crisis response, and long‑term goal setting. The Case Manager works closely with participants to ensure basic needs are met, safety plans are implemented, and pathways toward stability and self‑sufficiency are actively supported.

CCSI is partnering with Rise up Rochester, Inc. (RUR) to hire a Violence Prevention Case Manager. This position is employed by CCSI and working on behalf of Rise up Rochester, Inc. (RUR).In this role, you will Casework Management & Safety Planning:

Conduct comprehensive assessments of individuals impacted by violence to identify immediate safety concerns, housing needs, and long‑term support goals.

Develop, implement, and monitor individualized safety and case management plans.

Maintain consistent communication with participants to assess progress, emerging needs, and plan adherence.

Provide wrap‑around case management services that support stabilization and long‑term well‑being.

Accurately and consistently document all participant contacts, assessments, referrals, and progress notes in accordance with program and CCSI standards.

Housing Stabilization & Safety Monitoring:

Collaborate with community partners to identify and secure safe temporary and long‑term housing options.

Advocate on behalf of participants experiencing homelessness, unsafe living conditions, or displacement due to violence.

Conduct in‑person safety checks when participants are placed in temporary housing, including hotels.

Coordinate with outreach staff and hotel/property management to address safety concerns, property issues, and participant needs.

Support participants with housing‑related problem solving, conflict resolution, and tenancy stabilization.

Crisis Intervention & Basic Needs Support:

Provide direct crisis response and support for participants experiencing acute safety risks or destabilization.

Support participants in accessing food, transportation, medical care, clothing, and other basic necessities.

Provide approximately 20-30 hours per week of direct participant support focused on crisis response and stabilization.

Assist participants in navigating systems and reducing barriers to essential services

Community Based & Collaborative Practice:

Collaborate with community organizations, outreach teams, and service providers to support holistic care.

Participate in community events and violence prevention initiatives as appropriate.

Work collaboratively with internal RUR staff to ensure continuity and alignment of services.

Confidentiality, Ethics & Professional Practice:
