Position: CASE MANAGER & COMMUNITY SUPPORT SPECIALIST

Case Manager & Community Support Specialist

Provides intake and case management services, along with community support, including transportation via program van and assistance with life skills classes and resource coordination. Maintains confidentiality and ethical standards as identified by NASW, HIPAA or other relevant governing bodies. Utilizes Shahbaz philosophy to effectively support ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment for up to 45 Marketplace Apartments clients.

Essential Duties and Responsibilities

  • Maintains a caseload of 8-10 clients from the Marketplace Senior Apartment 55+ program
  • Conducting intake assessment and services to clients referred to Marketplace Apartments, self-referrals and or partnering agencies.
  • Completes required intake paperwork and any relevant screenings.
  • Complete all client's paperwork and goals into the HMIS system upon initial intake
  • Makes referrals to housing, DHS, education, and civil legal issues regarding evictions.
  • Complete letters of requests/referral forms, applications, and other required documents of eligibility to other agencies and or organization to advocate for resources which the client is initiated to.
  • Guide clients to complete applications for financial assistance, finding the best fit among community funders based on the client's needs, goals, and abilities
  • May deliver instructions during life skills classes in various topics that support client's ability to live independently
  • Serves as Case Coordinator for clients for program care. Works with the Nurse, Peer, and other stakeholders to ensure clients' needs are met. Coordinates and participates in regular case conferences.
  • Monitors and ensures achievement of program milestones and client outcomes.
  • Creates and implements life skills training and education opportunities for tenants.
  • Maintains client case files in accordance with Agency, departmental, regulator and funder guidelines.
  • Oversees client recordkeeping in compliance with appropriate regulatory agencies
  • Completes relevant screenings and assessments such as but not limited to psychosocial, mental health, financial status, and employment status.
  • Creates, implements, and monitors comprehensive, person-centered, trauma informed and solution-focused plans and goals. Works with clients on harm reduction and other related goals.
  • Assists in obtaining support for clients to include but not limited to home care, medical, dental, legal, substance use treatment, mental health, Department of Human Services, clothing and other entitlement needs.
  • Assist the client in understanding and meeting expectations of service providers, courts, and employers.
  • Inform clients of opportunities for social support such as recreation, faith-based, or self-help involvement. Make available tickets, transportation, and financial assistance where appropriate.
  • Provide ongoing evaluation, supervision, and development of program activities to ensure quality and effectiveness of services provided.
  • Implements and maintain procedures related to program operations.
  • Submits required reports and statistics in a timely manner.
  • Provides community support, including transportation via program van and establishing community resources and connections to community programs in collaboration with Marketplace Senior Apartments staff and leadership.
  • Transport program clients to necessary appointments and program activities
  • Keep clients and staff updated on available community resources
  • Provide information to clients and their service providers
  • Communicates with appropriate program partners, agency staff, community, and criminal justice agencies to ensure ongoing and effective referrals into programs.
  • Completes and maintains accurate van activity records such as but not limited to the following:
  • Accurate mileage and time sheet records
  • Reports problems, incidents to supervisor in a timely manner
  • Up-to-date client service schedule and accurate records trips and contacts
  • Facilitates vehicle up-keep and repair to include, but not limited to, inspections and oil changes.
  • Collaborates with referring agencies, funders and all related internal staff to effectively support individual client needs.
  • Maintains regular contact to coordinate client benefits and address all other treatment needs.
  • Reports to service providers about significant individual behavior or events that will require an immediate response.
  • Acquire concrete resources to meet individual needs.
  • Complete letters of requests/referral forms, and other required documents of eligibility or charitable organizations and public agencies to advocate for resources that the client is entitled to or eligible for.
  • Makes referrals for education, employment, legal issues, and others as needed.
