Delphi Rise is seeking a compassionate case manager to support individuals returning to the community from incarceration in Monroe County. You will assist with housing, employment, health care, and other supports using a person-centered approach to help clients rebuild their lives.
Responsibilities include coordinating with Parole and community partners, maintaining records, developing service plans, and facilitating support groups.
