Position Overview:
Job Title: Coordinator of Self-Direction Services
Location: Rochester, NY
Department: Community Services
Reports To: Senior Coordinator of Community
Employment Type: Full-Time, 40 hours a week - Monday-Friday between the hours of 7 am-5:30 PM; some flexibility is required.
Starting Wage: $24.62 - $28.73 per hour

Your
Core Responsibilities:
- Act as the main liaison between individuals supported, families, DDRO, OPWDD, Housing Subsidy agencies, and Fiscal Intermediary (FI) agencies.
- Supervise and provide ongoing support to assigned Brokers, FIs, and Housing Subsidy Liaisons; track training and ensure certifications remain current.
- Provide orientation and ongoing training to Self‑Hired staff and individuals supported as needed.
- Support community outreach efforts, attend and host informational sessions, and meet one‑on‑one with potential Self‑Direction participants and their families.
- Coordinate timely billing and reimbursements for Brokering, FI services, and Housing Subsidies in alignment with OPWDD and agency requirements.
- Ensure that all services provided are aligned with approved PRA budgets and Life Plans; monitor spending and offer feedback to Brokers and FI staff.
- Support a caseload and assist with payroll, self‑direction tasks, training needs, and agency projects.
- Address follow‑up on any health and safety concerns related to Housing Subsidy, FI, or Broker services, ensuring regulatory compliance.
- Maintain up‑to‑date documentation, including Life Plans, DDP1, DDP2, LCED, and Housing documents per OMIG, OPWDD, and agency guidelines.
- Conduct routine satisfaction checks, including home visits and phone calls, and take action to resolve any issues.
- Collaborate and communicate effectively with all stakeholders including supported individuals, families, DDRO, service providers, and agency staff.
- Maintain a professional appearance and model behaviors that support inclusive community living.
- Participate in audits, confirm vendor credentialing, complete reports, serve on committees, and perform additional duties as assigned.
What You Bring:
- Minimum of 1 year of experience working with individuals with developmental disabilities.
- At least 2 years of supervisory experience preferred.
- Valid New York State Driver’s License and reliable transportation.
- Ability to meet Lifetime Assistance driving and transport requirements.
- Strong leadership, organizational, and interpersonal skills.
- Commitment to quality service, regulatory compliance, and person‑centered support.
