US is seeking a Case Manager for its homeless prevention program located at Third Avenue, New York, NY 10035. The role provides comprehensive case management to an assigned caseload.
You will conduct intake within 24 hours, document services, and coordinate referrals to medical, mental health, and social services. Occasional weekend/evening hours and participation in group activities are expected.
