Schedule: Monday-Friday, 9:00 am–5:00 pm
Salary: $22.00–$23.50 per hour

About

The Role

The Steps to Success program is grounded in the nationally recognized EMPath Mobility Mentoring® model. The Mobility Mentor partners with enrolled families for up to two years, fostering trust and participant‑driven progress across key life domains:
Housing, Social Networks, Health, Savings/debt, Education/skill development, and Employment. The role offers a holistic, strength‑based approach to build self‑efficacy and stability for low‑income families.

Essential Duties And Responsibilities Participant Recruitment & Engagement
  • Promote outreach strategies and participate in recruitment activities to achieve enrollment goals.
  • Conduct intake and selection processes for new participants.
  • Build and maintain long‑term, trusting relationships with participants.
Case Management & Goal Planning
  • Partner with participants to create individualized goals and action plans.
  • Provide ongoing case management across social determinants of health.
  • Monitor and evaluate participant progress and outcomes.
  • Facilitate referrals to community service providers and coordinate resources.
Job Readiness and Placement
  • Complete job readiness assessments and support job preparation.
  • Connect participants with workforce initiatives and local employment opportunities.
  • Promote employment retention through post‑employment services.
Program Facilitation & Participant Support
  • Conduct individual counseling sessions, group meetings, prevention education, and developmental workshops.
  • Support coordination of special events such as community learning and graduation ceremonies.
Data Management & Compliance
  • Maintain accurate, up‑to‑date participant and program records.
  • Utilize community‑wide software systems for referrals and service coordination.
  • Participate in case reviews and audits to ensure compliance with processes and standards.
  • Maintain strict confidentiality of client and program information.
Organizational Participation & Professional Development
  • Attend relevant agency meetings, trainings, and workshops.
  • Pursue ongoing professional development through learning opportunities and conferences.
  • Demonstrate alignment with the agency’s Mission, Vision, Values, and Strategic Plan.

Requirements Qualifications
  • Education: Bachelor’s degree preferred. Associate’s degree required, or equivalent combination of experience and education considered.
  • Experience: 3 years of mentoring/coaching and participant navigation in a human services program preferred.
  • Knowledge of local provider networks preferred.
  • Experience with participant needs assessment, soft‑skill assessments, career interest inventories, housing, high‑school equivalency, financial literacy, credit repair, and banking processes preferred.
  • Experience in educational, career, or vocational development preferred.
Other Qualifications Relationship Building & Community Engagement

Demonstrated ability to build and maintain effective relationships at all organizational levels and with partners across public and private sectors. Familiarity with the Greater Rochester area and collaborative community structures is highly valuable.

Cultural & Communication Competence

Strong verbal and written communication skills, excellent interpersonal and customer‑service abilities. Respect for diverse cultural backgrounds and ability to engage with varied groups. Bilingual proficiency in English/Spanish strongly preferred;
American Sign Language skills a plus.
