The Arc of Monroe in Rochester, NY is hiring a part-time Care Manager. You will use a person-centered model to coordinate care for Medicaid eligible members with chronic conditions, build a Plan of Care, and ensure coordinated services.
The goal is to keep members healthy, out of the ER, and out of the hospital, with transitions supported across providers. You will determine eligibility, perform assessments, make referrals, and liaise with caregivers and HIT-enabled systems to link services.
