Location: City of Rochester

The primary purpose of this position is to coordinate and manage entitlements, including representative payee accounts and benefits, for individuals living in Empowering People’s Independence (EPI) residential homes.

Hiring Range: $26.75 – $28.00 Commensurate with Experience

Essential Functions:
  • Develops and maintains an understanding of regulations related to Medicaid, Medicare, the Social Security Administration (SSA), the State Supplement Program (SSP), the Supplemental Nutrition Assistance Program (SNAP), and Personal Allowance. Monitors SSA, SSP and SNAP eligibility for individuals living in residential homes.
  • Assists with the transfer of benefits during the intake and discharge processes for individuals living in residential homes.
  • Communicates updates and educational materials regarding individuals’ benefits and savings needs to supervisors, guardians, and other interested parties.
  • Ensures that individuals living in EPI’s residential homes receive timely and accurate financial information.
  • Processes and tracks Medicaid, SSI, and SNAP recertifications.
  • Reconciles representative payee accounts monthly and ensures that all checking account balances remain below established thresholds.
  • Invoices and tracks rent requirements for individuals living in EPI’s residential homes.
  • Performs periodic visits to residential homes and day programs to conduct onsite reviews and training with EPI staff.
  • Completes applications and forms required for individuals to receive accurate benefits and provides appropriate follow- up on outstanding requests.
  • Works collaboratively with residential management teams and provides frequent updates on the status of benefits.
  • Organizes and tracks financial records for approximately 80 individuals and escalates and tracks outstanding issues with the Director of Finance.
  • Performs other duties as assigned by the Director of Finance and/or designee within the scope of the position.

  • Participates in various EPI committees and department functions as able.
  • Participates in EPI events as able.

  • Must maintain confidentiality and exercise discretion concerning all business-related matters, including, but not limited to, board/vendors/donors’ names, investigations, finance, operations, and employee relations matters.
  • Must complete company required trainings mandated by Office for People with Developmental Disabilities (OPWDD), at time of hire and annually thereafter.
  • Must adhere to all departmental /company policies & procedures at all times.
  • Reports any incidents or compliance concerns to appropriate personnel.

Education and Experience:
  • Associate degree with two years of related experience required; bachelor’s degree with one year of related experience preferred. An equivalent combination of education and experience may be considered.
  • Experience as a care coordinator or care manager is preferred.
  • Knowledge of regulations related to Medicaid, Medicare, Social Security Administration (SSA), State Supplement Program (SSP), Supplemental Nutrition Assistance Program (SNAP), and Personal Allowance is preferred.
