Location: City of Rochester
Eagle Star Housing is seeking a dedicated case manager to support residents in a Permanent Supportive Housing program in Rochester, NY. You will provide case management, service plans, and coordination with outside providers to promote housing stability and self-sufficiency.
Under the Housing Director, you will conduct home visits, track progress, and assist residents with lease issues, income verification, and accessing community resources. Strong communication and teamwork are essential.
