North Central Family Medi cal Center is a non‑profit Federally Qualified Health Center providing affordable, accessible health care and pharmacy services to patients of all ages with locations in Rock Hill and Chester.
Nurse practitioners provide advanced primary and specialty nursing services including creating strategies to improve or manage a patient's health and introducing habits for health promotion.
Essential Duties and ResponsibilitiesOther duties may be assigned.
- Evaluates, examines and treats patients within scope of practice deemed appropriate by center affiliated professional organization.
- Obtains a complete medical history from patient, family, primary caretaker(s) and/or previous medical record.
- Performs complete physical examinations on patients with targeted physical exams when appropriate for isolated complaints.
- Orders routine and diagnostic lab and X-ray studies according to established criteria, when such studies are deemed appropriate for completing diagnosis and monitoring therapy, in consultation with a health center physician.
- Formulates, initiates and monitors patient management plans for each patient assigned to the patient panel of the provider based on established standards of care in consultation with a health center physician.
- Adjusts medications and other therapeutic regimens based on treatment goals.
- Conducts appropriate discussions of treatment plans with patients, family members and other health center personnel.
- Consults with physicians and other members of the health care team.
- Obtains appropriate referrals to specialist when appropriate.
- Interprets EKG's.
- Renders emergency care and treatment including cardiopulmonary resuscitation and sutures lacerations under the supervision of a health center physician.
- Participates in quality assurance program.
- Assists with development of annual health plan and its implementation.
- Provides medical consultation on an as needed basis.
- Uses EMR as appropriate to include messaging, billing, review of labs/notes and history documentation.
- Responsible for timely and accurate documentation of patient visits in EMR.
- Provides "after hours" call coverage which includes phone calls as required by the center.
- Provides coverage during times of staff shortages to include travel to other sites.
- Maintains confidentiality of all client information.
- Maintains all patient records in compliance with required standards and quality assurance protocols.
- Ensures all prescriptions and notes written by the NP/PA are reviewed by a health center physician.

Education/ExperienceSuccessful completion of a Board-certified nurse practitioner/physician assistant program which has been approved by the American Medical Association. Successful completion of the appropriate qualifying exam as required by the South Carolina Board of Medical Examiners.
Certificates and Licenses- Licensed by the South Carolina Board of Medical Examiners required
