Your Health Organization is seeking a dedicated Hospice Registered Nurse to join our Rock Hill-area team. As a Nurse Case Manager, you will visit patients at home and in facilities, coordinate with physicians, and help initiate telemedicine visits to support quality care.
Travel is required daily with shift patterns such as 8-hr days; you will partner with the care team to document plans, ensure regulatory compliance, and deliver compassionate, patient-centered care across our service area.
