About This Role

Highlights- Pay: $60,522 - $129,615/year
- Location: Rock Hill, SC
- Schedule: Full-time
- Support patient assessment and prioritization for walk-in and telephonic triage.
- Execute protocol-driven chronic condition management.
- Perform skilled nursing procedures including wound care and medication administration.
- Provide comprehensive patient and caregiver education regarding chronic conditions.
- Conduct medication reviews within nurse-led visits.
- Conduct onsite testing and use patient data to identify gaps in preventative screenings.
- Review and act upon high-priority abnormal laboratory results.
- Partner with providers to manage complex care plans and coordinate transitions.
- Utilize standardized protocols for medication management and prescription renewals.
- Delegate tasks to medical assistants and phlebotomists as needed.
- Active, Unrestricted Registered Nurse Licensure in the state of residence
- 2+ years of healthcare experience, working as an RN preferred
- BLS or CPR Certification required
- Electronic Medical Record (EMR) experience required
