Independent Living Coordinator
Full-Time | Exempt | Rock Hill, SC
Children's Attention Home is seeking a compassionate, organized, and dependable professional to serve as our Independent Living Coordinator
. This position provides direct support and consistent oversight within our Independent Living Program, helping young adults ages 18–21 develop the skills needed to transition successfully into adulthood.
The Independent Living Coordinator maintains an active presence within the Independent Living community and provides individualized, person-centered support to program participants. Working closely with the Independent Living Manager, this position assists with daily program operations, coordinates services and resources, facilitates independent living instruction, and helps ensure a safe, engaging, and supportive environment.
Key Responsibilities Include:- Providing direct support, case management, assessments, and individualized service planning for participants
- Helping participants build independent living skills related to education, employment, finances, housing, health, relationships, and daily living
- Facilitating house meetings, workshops, and Independent Living curriculum
- Coordinating services and resources with DSS, community partners, educational institutions, employers, healthcare providers, and other support networks
- Maintaining accurate participant records, progress notes, service plans, and required documentation
- Assisting with participant onboarding, daily schedules, transportation, activities, crisis response, on-call support, and community operations
- Bachelor's degree in a human services or related field
- Experience working with young adults, child welfare populations, or a similar setting
- Minimum of two years of supervisory experience
- Valid driver's license and ability to meet agency training requirements
