The compensation range reflects a good faith estimate of starting base pay for full-time employment at the time of posting. Final pay may vary by location and individual qualifications. $85,400 - $117,500 per year.

• Plan and administer prescribed skilled physical therapy treatment and training for patients to attain the highest level of physical function.
• Test and screen patients to assist physicians in evaluating function and develop treatment objectives.
• Develop and implement conditioning/rehabilitation programs aligned with the physician’s Plan of Treatment and patient goals; adjust treatment as needed.
• Collaborate with physicians and clinical team to obtain information and revise treatment plans.
• Provide instruction and supervision to Physical Therapy Assistants and Home Health Aide staff; monitor documentation and ensure plan-of-care alignment.
• Document care observations, interventions and evaluations promptly and accurately; process orders and ensure physician signatures as required.
• Report patient progress to physicians, Clinical Manager, staff, patients and families; submit evaluation, treatment plans, progress reports and discharge summaries as required.

• Degree from an accredited Physical Therapy Program (APTA approved)
• Minimum of one year physical therapy experience preferred
• Current and unrestricted Physical Therapy license
• Current CPR certification
• Strong organizational and communication skills
