Oak Street Health seeks a Behavioral Health Specialist Fellow to support adult Medicare patients in an integrated primary care setting. In this fellowship role, you will deliver brief behavioral interventions, conduct psychosocial assessments, and support patients managing mental health and chronic conditions. You’ll collaborate closely with an interdisciplinary care team, coordinate referrals to community resources, and contribute to preventative, patient-centered care.

- Provide brief behavioral health interventions and counseling to adult Medicare patients within a primary care setting.
- Collaborate with physicians, nurses, and care teams to develop and implement integrated care plans.
- Conduct psychosocial assessments, screenings, and risk evaluations for mental health and substance use concerns.
- Offer patient and family education on coping skills, behavior change, and treatment adherence.
- Coordinate referrals to community resources, specialty mental health, and social services.
- Track patient progress, document encounters, and maintain accurate, timely records in the Electronic Health Record (EHR).
- Participate in case conferences, huddles, and quality improvement initiatives to enhance patient outcomes.
- Support crisis intervention, safety planning, and warm handoffs when patients present with acute needs.
- Provide brief behavioral interventions such as CBT and motivational interviewing.
- Perform psychosocial assessment and risk screening.
- Manage care coordination and referrals.
- Document in the Electronic Health Record (EHR).
- Conduct crisis assessment and de-escalation.
- Leverage knowledge of community and social service resources.
- Educate patients and families.
- Collaborate with the interdisciplinary team.
- Support population health and chronic disease initiatives.
- Participate in data-informed quality improvement efforts.
- Licensed or license-eligible behavioral health credential (e.g., LCSW, LPC, LMFT) preferred.
- Master’s degree in social work, counseling, psychology, or related field strongly preferred.
- Training in CBT, motivational interviewing, or brief therapy models is a plus.
- Certification in crisis intervention or de-escalation techniques beneficial.
