Care Diem Home Health is seeking compassionate and reliable Home Health Aides to provide services to patients in Rockford
. Our mission is to help clients remain safely at home while receiving top-quality, personalized care.
- Provides Personal Care
- Perform simple procedures as an extension of therapeutic services
- Ambulation and exercise of the patient
- Household services essential to health care at home
- Assistance with medications that are ordinarily self-administered
- Reporting changes in the patient's condition and needs to the registered nurse
- Completion of appropriate clinical records to record care given including observations and treatments.
- Training as a Home Health Aide and completed competency evaluation is required
- HHA must currently be on the Illinois State Registry
- 1+ year experience (home health preferred)
- CPR certification (BLS)
- Reliable transportation, valid driver's license, and auto insurance
- Strong judgement, communication, and independence
- Flexible scheduling (FT, PT, PRN, Per Visit Based)
