We are seeking a dedicated and compassionate Wound Care Coordinator to join our care team. The ideal candidate will be a proactive team player with strong clinical skills and a commitment to quality patient care.

• Assess, plan, and implement individualized wound care plans for residents.
• Monitor wound progress and adjust treatment strategies as needed to ensure optimal healing
• Collaborate with interdisciplinary team members, including physicians, therapists, and other healthcare professionals
• Educate residents, families, and staff on wound prevention and care techniques
• Maintain accurate documentation of wound assessments, treatments, and progress notes
• Participate in on-call responsibilities to provide wound care support outside of regular hours
• Ensure compliance with healthcare regulations and facility policies related to wound management

• Valid Registered Nurse (RN) license in the state of practice
• Prior experience in wound care preferred
• Wound Care Certification (such as CWCN or WCC) is a plus
• Strong assessment and clinical skills with attention to detail
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Flexibility to participate in on-call duties as required
