Coordinates nursing services for assigned individuals in the Residential Services program, including the assessment and ongoing monitoring of the health status of assigned individuals; developing and overseeing the delivery of individualized care plans and medication administration; maintaining working relationships with community-based health care providers; and, facilitating the completion of routine administrative duties such as purchasing, billing, training, policy/procedure writing, and documentation.

Prepares reports/data as requested by the Executive Leadership. Performs routine nursing duties as an RN. Provides direct supervision of assigned RN staff. Will follow Rule 50, Dept. of Human Services Rule 115, Rule 116, Rule 119, Rule 120, and other relevant regulatory guidelines.

Essential Duties/Responsibilities

  • Support the philosophy through the agency’s mission, vision, and values.
  • Responsible for leading the achievement of the organizational objectives as assigned in the agency’s Strategic Plan.
  • Serves as a leader and health services subject matter expert, thorough working knowledge of the principles and practices of intervention, prevention, support, case management and/or other services for developmentally disabled populations.
  • Supervise the performance of all RNs, providing training and support as needed. Complete performance evaluations, review and authorize paid leave requests, document absences, and travel expenses.
  • Assist in the development and implementation of operating budget, long—and short-term planning, and service evaluation methods. Assist in the assessment of resource requirements, including staffing, equipment, and supplies.
  • Work cooperatively to link the department with services and resources that have the potential to improve the delivery of health services. Collaborates with internal peer leaders to assure seamless care delivery and medication management/administration.
  • Successfully completes the DHS nurse-trainer (RN-T) program, oversees RN-T staff, and the process and monitoring associated with the delegated medication administration (initial authorizations and re-authorizations as needed/directed).
  • As needed, order and secure medications and medical supplies.
  • As needed, schedule auxiliary medical services.
  • Manages communications/updates related to new prescriptions for individuals, medication changes, and/or new orders for testing.
  • Monitor RNs, LPNs, and other personnel involved in implementing and maintaining health care services, ensuring that appropriate and high quality services are being delivered. As part of the community support team, participate in developing and ongoing review of individualized service plans for individuals served.
  • Provide training, consultation, and support to individuals served, families, guardians, and staff as requested.
  • Assures that departmental procedures, policies, and training are up to date. Writes new procedures and protocols, as needed.
  • Works collaboratively with the QI (quality improvement)/safety department and monitors medication errors and develops and implements corrective action plans, as needed.
  • Prepares and analyzes data, as necessary, to monitor processes and clinical outcomes.
  • Ensure documentation and records are maintained by pertinent policies, procedures, Rule 116 and standards of practice. Key examples include MAR documentation (Medication Administration Records), SAMA (self-administration of medication assessments), and new/yearly health assessments for individuals served through the agency.
  • Participate in the rotating on-call system, interpreting symptoms and providing direction to staff and other professionals as needed.
  • Facilitates with guiding EP!

    C teams (such as QIDPs) with ensuring that individuals and guardians are informed and educated in a timely manner regarding individual care plans and any changes in the care plans.
  • Follows all HIPAA regulations and ensures departmental compliance.
  • Marginal Responsibilities
