Essential Duties and Responsibilities- Assess patient data and performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician
- Maintain timely consult schedule.
- Provide patient and family education to assist patient and family to understand role of palliative care in current health situation.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient clinical problems and health care needs if required.
- Record physical findings, and formulate plan and prognosis, based on patient condition.
- Coordinates care with Physician and other health professionals to prepare comprehensive patient care plan.
- Discusses health care plan and goals of individual patients for periodic review and evaluation by Physician.
- Refers patients to Physician for consultation or to specialized health resources for treatment.
- Assists in development of protocols.
- Graduate of an accredited nursing program.
- Graduate of an accredited masters of nursing program.
- Meets the requirements determined by the State of Wisconsin or Illinois to perform duties defined as those of a Nurse Practitioner.
- Hold a valid APRN & RN license in state of practice.
- Hold a valid DEA license in state of practice.
- CPR or BLS certification or other certifications as required
