Nurse Practitioner – Geriatrics No Call

A specialized infectious disease practice serving skilled nursing and long-term care facilities across Chicago's north suburbs is seeking a dedicated Nurse Practitioner (AGNP, ANP, DNP, or FNP) for a full-time multi-site role covering Evanston, Skokie, Rogers Park, and Northbrook. This is a mission-driven, Monday–Friday position with no nights, no weekends, and flexible daily start times — built around proactive infectious disease management, reducing rehospitalizations, and building meaningful clinical relationships with a geriatric long-term care population that genuinely depends on consistent, expert-level care.

• Diagnose, treat, and manage infectious diseases across skilled nursing and long-term care facilities
• Perform comprehensive physical assessments and monitor changes in patient conditions
• Prescribe medications and develop individualized treatment plans
• Participate in and guide residents' plans of care
• Coordinate advanced care planning and discharge processes
• Facilitate referrals and collaborate with physicians and specialists
• Maintain a minimum of 12 patient visits per day (bonus eligible for exceeding targets)
• Ensure timely, accurate documentation and communication across facilities

• MSN from accredited NP program required
• 1–3 years NP experience; infectious disease experience strongly preferred
• Experience in acute, sub-acute, or long-term care settings required
• Background in geriatrics, internal medicine, primary care, hospice, or palliative care preferred
• Knowledge of Medicare, Medicaid, and managed care structures a plus
• Strong interpersonal skills with elderly populations

• $50–$60/hour + monthly performance bonus
