Licensed Practical Nurse II

Licensed Practical Nurse II is accountable for the delivery of patient care as delegated by and under the direction of a physician or registered nurse (RN) utilizing the Nursing Process and Relationship-Based Care model as the framework. The Licensed Practical Nurse II provides direct patient care and takes initiative to improve patient outcomes by integrating Shared Leadership, Evidence Based Practice, Relationship Based Care and standards of nursing excellence into clinical practice.

Through therapeutic relationships with patients, families, self and colleagues, the Licensed Practical Nurse II functions as an interdisciplinary team member who provides quality patient care and fulfills unit and organizational goals.

Duties & Responsibilities

  • Carries out or assists with diagnostic procedures, which may include identifying and recording subjective symptoms, such as pain; observing and recording objective symptoms, such as body excreta and discharge, pulse, temperature, respiration, blood pressure, and changes in condition of body; collecting specimens for the laboratory tests, such as testing urine for blood sugar, acetone, and specific gravity.
  • Carries out therapeutic procedures, which may include applying binders and bandages, and applying heat or cold packs.
  • In accordance with established nursing policies and procedures, administers oral medications, and intra muscular, subcutaneous, and intra dermal injections. Performs intravenous therapy, as directed by an RN or physician.
  • Communicate with patients to assess emotional and mental well-being; identifies and reports patients requiring special assistance.
  • Charts and reports nursing care activities performed and reports observations made.
  • Performs audits of narcotics by counting supply on hand, comparing amount against record of use and reporting discrepancies.
  • Dispense medication (including methadone) in accordance with protocols set by IDPH, DBAR, DEA, and FDA.
  • Monitor the appropriateness of methadone and other medication dosages.
  • Observe and document clients' physical condition, including signs of intoxication or withdrawal, and consult with medical staff as needed.
  • Conduct initial medical assessments and maintain accurate documentation in client records.
  • Follow up on TB tests, reactive results, and any referrals or recommendations from medical visits.
  • Administer breathalyzer tests as required to determine recent substance use.
  • Maintain accurate medication records, including DBAR reports, methadone counts, and provider orders.
  • Ensure proper storage and safekeeping of controlled substances and prescription medications.
  • Collaborate with medical providers and counselors to support integrated care planning.
  • Participate in daily clinical rounds, utilization reviews, and quality assurance initiatives.
  • Complete thorough and timely nursing notes and documentation for all patient interactions.
  • Perform supportive tasks related to nursing care, which may be adjusted or reassigned based on program or patient needs. Maintain professional and ethical standards in all interactions with clients and staff.
  • Stay informed of updates in substance use treatment protocols and relevant nursing practices. Must work effectively with diverse individuals and organizations across cultural, generational, ethnic, and professional backgrounds.
  • Performs other related duties as assigned.

Minimum Qualifications

  • Possession of a valid license as a Licensed Practical Nurse issued by the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.
  • Six months of nursing experience in a health care facility (such as a hospital, nursing home, medical clinic, prison, mental health facility, or closely related facilities).
  • Current AHA BLS/CPR and other required unit specific certifications
  • Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, family, and health care providers
  • Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR) with on-the-job training
  • Excellent attendance, interpersonal skills and work history required.
  • Preferred Qualifications

    One year LPN experience preferred. Demonstrated knowledge of substance use disorders, detoxification, and recovery processes. Understanding of general nursing practices and medical terminology. Working knowledge of Illinois state and federal regulations applicable to behavioral health and medication-assisted treatment (MAT). Strong written and verbal communication skills required. Employee must be able to utilize a computer, phone, etc. for extended period of time and write or type notes for at least two (2) to four (4) hours per day.
