LPN, LPN​/LVN, Healthcare Nursing

ESSENTIAL DUTIES AND RESPONSIBILITIES Rooms patients, collects assessment data and prepares patient according to specific need.

Collects patient data and documents in the medical record.

Prepares and maintains patient charts.

Assists in examination, treatment and other clinic procedures.

Administers injections and medications as ordered and directed.

Reviews physician's orders, lab requests, or follow-up needs with patient.

Assists with laboratory procedures, screening tests and QI projects

Answers telephone and either responds to inquiry or directs caller to appropriate personnel for triage

Documents according to standards of care.

Provides patient treatments as ordered.

Coordinates referrals for patients through insurance and other physician offices

Observes patients and reports adverse reactions to medication or treatment to the provider orRN.Perform the duties of a clinic LPN within alignment to LPN scope of practice.

Assist with clinic projects and assignments and nursing initiatives.

May provide feedback for evaluation or participate in peer review.

Assists with preparation of staff for surveys.

Monitors compliance with clinic initiatives as assigned

Administers specified medications and nursing interventions as allowed by policy and procedure, licensure, and State Practice Act.

Reinforces patient teaching provided by Provider and RN.Provides basic healthcare instruction to family and patient.

Performs EKG's and simple lab tests as needed.

Performs venipuncture to collect blood specimens.

Participates in QA & I.Orders and maintains supplies.

EDUCATION AND/OR EXPERIENCE Graduate of an accredited LPN Program OR Wisconsin:
Have completed the portion of the RN Program needed to obtain a certificate of completion in practical nursing.

Illinois:
Have completed the designated curriculum of a Ladder Option Approved Designated RN Program.

Experience in clinical setting preferred.

CERTIFICATION/LICENSURELicensed as an LPN in the State of Practice Approved CPR or BLS certification within 90 daysOTHER

N/ALEVEL OF SUPERVISION This job has no supervisory responsibilities. Work is performed according to specific procedures and methods, but the incumbent must make minor decisions in order to adapt to routine situations.

SUPERVISE SN/APAY RANGE:$22.28 - $35.66
