St. Croix Hospice is seeking a Hospice Transition Nurse to serve as a resource RN for patients, families, hospitals, physicians and nursing homes.
The role focuses on sustaining and growing referrals from hospitals and nursing homes, guiding admissions, GIP patients, and smooth transitions for patients and families. The nurse will coordinate transitions from the hospital to home or hospice care, educate referral sources about programs, and mentor other RNs for high-acuity needs.
