Rockford Nephrology Associates (RNA)is growing and looking for a motivated, compassionate APP's to join our collaborative nephrology team. This is an excellent opportunity for an experienced nephrology provider—or an APP ready to develop specialty expertise in a supportive environment.

no weekends and no call

What You'll Do

As a Nurse Practitioner at RNA, you'll provide comprehensive care to patients with kidney disease across multiple clinical settings.

Outpatient Nephrology
  • Evaluate and manage patients with chronic kidney disease and related conditions.

  • Perform comprehensive histories and physical examinations.

  • Develop individualized treatment plans and monitor patient progress.

  • Order and interpret laboratory and diagnostic studies.

  • Manage medications and coordinate referrals and consultations.

  • Educate patients on kidney health, disease management, treatment options, and prevention.

  • Help patients take an active role in their long-term health.

Dialysis Care
  • Evaluate and manage patients receiving hemodialysis and/or peritoneal dialysis.

  • Monitor and address common complications of ESRD, including anemia, hypertension, fluid and electrolyte abnormalities, bone and mineral disorders, and vascular access concerns.

  • Evaluate treatment effectiveness and adjust plans of care in collaboration with the nephrology team.

  • Coordinate care between the dialysis center, outpatient clinic, hospital, and other healthcare providers.

  • Provide education and support to patients and their families.

Hospital/Inpatient Care
  • Perform patient assessments and histories and participate in inpatient nephrology management.

  • Manage acute and chronic kidney-related conditions.

  • Develop and monitor treatment plans.

  • Participate in discharge planning and coordinate appropriate follow-up.

  • Educate patients and families regarding treatment plans and expectations.

  • Ensure continuity of care following hospitalization.

Compensation & Benefits
  • Starting salary: $130,000+, negotiable based on experience

  • $1,500 annual CME allowance

  • 40 hours of CME time

  • Generous accrued PTO

  • 40 hours of Illinois paid leave

What We're Looking For
  • Master's or doctoral degree from an accredited Nurse Practitioner program or Physician Assistant Studies Program.

  • Current Illinois NP/PA license and national board certification

  • Strong clinical assessment, diagnostic, and treatment-planning skills

  • Excellent communication and interpersonal skills

  • Ability to work collaboratively with physicians and multidisciplinary teams

  • Commitment to providing compassionate, patient-centered care

  • Nephrology, dialysis, hospital medicine, or other relevant clinical experience is preferred, but motivated NPs interested in developing nephrology expertise are encouraged to apply
