• Collaborate and inform the case management process.
• Assist with appointment scheduling for members.
• Identify and assist members in accessing benefits and education.
• Utilize care management tools and resources effectively.
  
  Work independently and utilize virtual collaboration tools.
• Demonstrate proficiency with standard corporate software applications.
• Analyze and solve problems effectively.
• Communicate effectively with members, providers, and community organizations.

• Associate's degree required, Bachelor's preferred

• Minimum 3-5 years clinical practical experience
• 2-3 years in Care Management, discharge planning, and/or home health care coordination preferred

• Active and unencumbered Registered Nurse license in the state of Illinois required
• Certified Case Manager preferred
