You will perform a detailed initial assessment of the patient, family, and home environment to determine specific home health care needs, monitor nursing status, and initiate the plan of care as patient status changes.
Requires current Illinois RN licensure, CPR, a valid IL driver’s license with auto insurance, and preferably Home Health experience; HCHB
