Palliative Care Social Worker

Northern Illinois Hospice seeks a compassionate Palliative Care Social Worker to support patients and families facing serious illness in Rockford and surrounding communities. In this role, you will provide psychosocial assessments, counseling, advance care planning, and resources that honor patient goals, values, and culture. As a key member of our interdisciplinary team, you will collaborate with nurses, physicians, chaplains, and volunteers to coordinate care, advocate for patient needs, and ease complex transitions.
