Position
Community Health Nurse - VNA Team 2
OverviewThe community health nurse is a professional nurse knowledgeable in community health nursing who provides nursing care to patients in the community setting.
CompensationPay Range: $70,699.20 - $
ScheduleMonday to Friday 8:00-4:30, 1 weekend every 4 weeks, 1 holiday per year.
Responsibilities- Schedule and prioritize nursing visits according to client acuity and the organization’s emergency preparedness plan. Create weekly schedules and make scheduled home visits per assignment.
- For each admission to caseload, develop an individualized and holistic plan of care in collaboration with patient/family/caregiver. Perform admission physical and psychosocial assessment to determine appropriate plan of care.
- For each visit, implement the plan of care, nursing interventions, and procedures necessary for the skilled care of the patient. Update the plan on an ongoing basis as goals are met and problems identified.
- For each discharge, demonstrate improved outcomes and ensure all disciplines involved agree on the plan. Prepare patient for discharge appropriately.
- Administer and document medications. Complete medication reconciliation every visit and update medication profile with changes as appropriate. Run DRR for each additional medication and educate patient/family about medications and side effects.
- Coordinate the plan of care for each patient on caseload, involving patient/family in care planning and communicating with all disciplines to ensure quality of care.
- Foster a culture of safety, comply with hand hygiene and infection control guidelines.
- Participate in opportunities to maintain, enhance, and promote the image of the professional nurse.
- Serve in a formal or informal leadership role.
- Demonstrate use of advanced clinical skills and knowledge, incorporating evidence‑based practice and research into clinical practice.
- Graduate of an accredited School of Nursing.
- Bachelor of Science degree in nursing from a NLN‑accredited institution (preferred).
- One to two years of medical/surgical nursing experience (preferred).
- Valid RN license in Massachusetts.
- Strong written, verbal and reading English communication skills.
- Basic nursing competencies.
