The community health nurse is a professional nurse knowledgeable in community health nursing who provides nursing care to patients in the community setting.
Compensation Pay Range: $70,699.20 - $
Responsibilities- Complete weekly schedule and make scheduled home visits per assignment. Schedule and prioritize nursing visits according to client acuity and in accordance with the organization’s emergency preparedness plan.
- For each admission to caseload, develop an individualized and holistic plan of care for the patient in collaboration with the patient/family/caregiver. Perform admission physical and psychosocial assessment to determine appropriate plan of care.
- For each visit, implement the plan of care, nursing interventions, and procedures necessary for skilled care of the patient, and update the plan of care on an ongoing basis as goals are met and problems are identified.
- For each discharge, demonstrate improved outcomes in documentation; prepare patients for discharge and ensure agreement among all disciplines involved in the patient’s care.
- Administer and document medications, completing medication reconciliation at each visit, reviewing and updating medication profiles, running DRR with each additional medication, and teaching medication management to patients/families.
- Coordinate the plan of care as a case manager, involving the patient/family in care planning and communicating with all disciplines to ensure quality of care.
- Foster a culture of safety through ownership and commitment to a safe environment, complying with hand hygiene and infection control guidelines.
- Participate in opportunities to maintain, enhance, and promote the image of the professional nurse.
- Serve in a formal or informal leadership role.
- Demonstrate advanced clinical skills and knowledge, incorporating evidence‑based practice and research into clinical practice.
M-F 8:00‑4:30, 1 weekend every 4th weekend, 1 holiday per year.
Qualifications- Graduate of an accredited School of Nursing.
- Bachelor of Science degree in Nursing from a college/university accredited by the NLN is preferred.
- One to two (1‑2) years of medical/surgical nursing experience is preferred.
- Proficient in written, spoken, and understood English; basic nursing competencies.
- Licensed as an RN in the State of Massachusetts.
