Rockledge Health & Rehabilitation Center is looking for an excellent Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join their Team as the Wound Care Nurse.
Position Summary
The RN/LPN Wound Care Nurse is responsible for coordinating and providing specialized wound, skin, and ostomy care to residents in accordance with physician orders, facility policies, Florida nursing regulations, and current evidence-based wound care practices. This position works collaboratively with the interdisciplinary team to promote wound healing, prevent skin breakdown, reduce facility-acquired pressure injuries, and ensure compliance with federal and state regulations.

Perform comprehensive skin and wound assessments upon admission and as indicated.
Assess, stage, measure, photograph (per facility policy), and document wounds accurately.
Develop, implement, and evaluate individualized wound treatment plans.
Perform wound treatments and dressing changes utilizing aseptic and sterile techniques.
Monitor healing progress and promptly communicate changes in resident condition to the physician or advanced practice provider.
Participate in weekly wound rounds with the interdisciplinary team.
Collaborate with physicians, nurse practitioners, dietitians, rehabilitation staff, and nursing personnel regarding wound management.
Ensure physician orders for wound care are current, complete, and accurately implemented.
Recommend appropriate pressure-relieving devices, specialty mattresses, cushions, and positioning interventions.
Monitor residents for signs of infection and initiate appropriate interventions.
Educate residents, families, and staff regarding wound prevention and treatment.
Assist with prevention programs for pressure injuries, skin tears, diabetic ulcers, venous ulcers, arterial ulcers, and surgical wounds.
Monitor nutritional needs related to wound healing and collaborate with dietary services.
Maintain accurate documentation in the electronic medical record in accordance with CMS, Florida regulations, and facility policy.
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives related to skin integrity and wound care.
Assist with survey preparation and participate during state and federal surveys.
Track wound outcomes and quality indicators.
Ensure appropriate wound care supplies are available and assist with inventory management.
Follow infection prevention and control practices at all times.
Serve as a clinical resource for nursing staff regarding skin and wound care.
Perform other nursing duties as assigned.
Current, active Florida Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.
Graduate of an accredited nursing program.
Current CPR/BLS certification.
Minimum one (1) year of nursing experience in a skilled nursing facility, long-term care, rehabilitation, or acute care setting.
Knowledge of wound assessment, treatment modalities, pressure injury prevention, and infection prevention practices.
Excellent assessment, communication, and organizational skills.
Ability to maintain confidentiality in accordance with HIPAA.
Wound Care Certification (WCC, WOCN, CWCN, or equivalent), or willingness to obtain within a designated timeframe.
