State Opioid Response (SOR) Specialist

Salary: $52,000

The State Opioid Response (SOR) Specialist serves as the primary point of contact and liaison for the State Opioid Response (SOR) Program, supporting families involved in the child welfare system who are affected by opioid use disorders. The SOR Specialist provides family engagement, care coordination, referral management, and linkage to appropriate substance use treatment services while collaborating with multidisciplinary partners throughout the Brevard System of Care.

This position is responsible for coordinating treatment services, facilitating collaborative planning, monitoring participant progress and program outcomes, and ensuring compliance with grant requirements and program deliverables. The SOR Specialist works closely with community providers, child welfare professionals, court personnel, and behavioral health partners to promote positive outcomes for children and families. The position is accountable for accurate documentation, data management, outcome tracking, reporting requirements, confidentiality, and adherence to agency policies and State Opioid Response grant expectations.

Technical/Functional Expectations

1. State Opioid Response (SOR) Program Monitoring – Essential Function

Serves as the primary point of contact and liaison for the State Opioid Response (SOR) Program. Monitors participant engagement, treatment progress, program outcomes, and adherence to grant deliverables. Maintains accurate program data and supports continuous quality improvement through ongoing monitoring and performance evaluation.

2. Treatment Coordination and Referral Management – Essential Function

Provides outreach and engagement to families and collaborates with providers, child welfare professionals, and community partners to coordinate referrals, facilitate access to substance use treatment services, and ensure timely linkage to appropriate behavioral health and recovery supports.

3. Multidisciplinary Collaboration and Treatment Planning – Essential Function

Coordinates multidisciplinary staffing meetings to promote collaborative case planning, treatment coordination, and family-centered decision-making. Works closely with treatment providers, child welfare professionals, and community stakeholders to support positive family outcomes and continuity of care.

4. Facilitates Family Team Conferences – Essential Function

Facilitates Family Team Conferences utilizing Wraparound principles, including family voice and choice, team-based planning, natural supports, collaboration, community-based services, cultural responsiveness, individualized planning, and strengths-based practice to develop comprehensive service plans.

5. Dependency Court and Early Childhood Court Coordination – Essential Function

Coordinates with Dependency Court and Early Childhood Court stakeholders to support case planning, service coordination, and ongoing monitoring of families participating in the State Opioid Response Program. Collaborates with judges, attorneys, child welfare professionals, service providers, and case management staff to ensure coordinated service delivery and successful program outcomes.

6. Documentation, Data Management, and Reporting – Essential Function

Maintains timely and accurate case documentation, client records, and program data in accordance with agency policies, confidentiality requirements, and applicable federal and state regulations. Collects and analyzes program data, monitors outcomes, prepares required grant reports, and ensures compliance with State Opioid Response grant deliverables and documentation standards.

7. On-Call Rotation – Essential Function

Participates in the agency's on-call rotation as assigned.

8. Other Duties as Assigned

Performs other related duties as assigned, including assisting with special projects, supporting agency initiatives, training staff when appropriate, and performing higher-level responsibilities for developmental purposes.
