Position Overview
This is a full‑time Registered Nurse, Home Health position located in Corpus Christi, TX. Eligible candidates receive a $5,000 Sign‑On Bonus, hourly pay ranging from $28.94 to $51.63, and benefits within 30 days, including PTO, scrubs, annual bonus potential, annual reviews, tuition and mileage reimbursement. The role requires travel to patient homes in the Corpus Christie area. Business hours are Monday‑Friday 8a.m.‑6p.m. with additional on‑call duty during the week and weekends on a rotating schedule.
Overtime and differential pay are available.
- Perform nursing assessments and develop, implement and evaluate plans of care in conjunction with physician, patient and caregivers per agency policy and procedure.
- Evaluate the effectiveness of the plan of care, the patient’s response to care and outcomes of care and revise the plan as needed.
- Re‑evaluate patient nursing needs to include ongoing assessments using the OASIS data set at appropriate time points.
- Provide skilled nursing services that require substantial and specialized nursing care including medication administration, wound care, therapeutic treatments and skilled teaching as ordered and indicated.
- Plan and implement patient health teaching and health counseling regarding the disease process(es), self‑care techniques and prevention, involving significant others in achieving health goals while promoting patient/family independence.
- Participate in health promotion and disease prevention activities.
- Initiate appropriate preventive and rehabilitative nursing procedures following the physician’s plan of care.
- Coordinate services and scheduling with the Clinical Team Manager and Clinical Team Coordinator, including recommendations for additional home health care services within 24 hours of Start of Care (SOC).
- Assess the patient’s condition during every home health visit; ensure assessments are communicated to the Clinical Team Manager daily; implement nursing interventions to meet patient needs and changing conditions.
- Perform care‑management duties on patients as assigned, including notifying the physician of changes in the patient’s condition or progress toward goals, obtaining needed physician orders, reassessing for recertification, supervising LVNs and home health aides, attending and documenting case conferences, and coordinating care by reporting significant findings to other team members.
- Document all communications with the patient, family, physician, CTM, pharmacy, and other disciplines on communication notes.
- Prepare a written plan for the Home Health Aide to follow, if applicable.
- Express verbal and written communication in a clear, positive and cooperative manner.
- Perform additional Home Health Aide duties as needed.
- Graduate of an accredited school of nursing.
- Current unrestricted Texas RN license or compact license.
- BLS certification or ability to obtain within 30 days of employment.
- 1+ year experience in home health care or a related environment.
- Wound care experience.
- Knowledgeable in Microsoft Office applications including Outlook, Word and Excel.
- Ability to read, analyze and interpret information in medical records and health plan documents.
- Driver’s license and reliable transportation to travel to client and/or patient sites within the designated area.
- Bilingual (Spanish).
- OASIS experience.
Ability to lift and transfer patients.
Compensation and BenefitsHourly pay ranges from $28.94 to $51.63 based on local labor markets, education, work experience and certifications.
