Adventist Health Care Rehabilitation seeks to hire a Nurse Practitioner Clinical Liaison who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing.

Responsibilities
  • Serve as the team’s primary point and decision maker for complex patients, clinical triage, and critical decision making; communicate with referral sources and Rehab physiatry as indicated.
  • Perform quality assurance audits of the Rehab Referral tracker to ensure the efficient processing of Rehab referrals.
  • Collaborate with the liaison(s) to conduct comprehensive pre‑admission evaluations, including chart reviews and face‑to‑face assessments, to determine eligibility for inpatient rehabilitation.
  • Build and maintain strong relationships with hospitals, physicians, discharge planners, and payors.
  • Promote facility services through educational presentations, tours, and outreach activities.
  • Provide updates to and facilitate obtaining feedback from referring providers and the Rehab Physiatry team.
  • Assist the BD Director in resolving barriers to admission and contributing to performance improvement initiatives.
Qualifications
  • Active Nurse Practitioner license in Maryland required.
  • Minimum 5 years in acute care; prior experience on an acute care hospitalist team strongly preferred. Prefer candidates with strong medical/surgical acute care and ICU experience.
  • Active American Heart Association Basic Life Support (BLS) certification required; ACLS preferred.
  • Valid driver’s license and ability to travel between hospitals.
  • Strong clinical judgment, communication, and organizational skills; proficiency with EMR systems; knowledge of Medicare/Medicaid and insurance guidelines.
  • Experience in marketing or business development within post‑acute care settings is a plus.

  • Employment Type: Full‑time
  • Hours per Week: 40 hrs a week

    Schedule:

    8:00 AM–4:30 PM Monday through Friday
  • Shift Type: Days/Evenings
  • Weekend Requirements: As needed
  • Holiday Requirements: As needed
  • Shift Differential Eligible: No

Hybrid schedule includes remote, in‑office and onsite hospitals.

Pay Range

Pay Range: $89,356.80 - $

If the salary range is listed as $0 or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.

Under the Fair Labor Standards Act (FLSA), this position is classified as: United States of America (Exempt)
