About the Position
Hospice Admissions Nurse (RN)
Location:
Rockville, MD
Job Type: Full-Time
Schedule:
4 days per week | 10-hour shifts (8:30 AM 7:00 PM)
Pay Rate: $45/hour and up (based on experience)
Position Overview Nurse America is seeking a compassionate and experienced Hospice Admissions Nurse (RN) to join our dedicated hospice team serving patients throughout Montgomery County. This role focuses on conducting admissions, initiating plans of care, and delivering high-quality hospice and palliative services that promote dignity, comfort, and peace during end-of-life care.
The ideal candidate is clinically strong, empathetic, highly organized, and comfortable leading meaningful conversations with patients and families during critical moments.
Key Responsibilities
Conduct comprehensive initial assessments to determine patient eligibility and care needs
Obtain informed consent and coordinate admission into hospice services
Develop and review individualized plans of care in collaboration with physicians
Serve as a clinical resource for referral sources, patients, and family members
Facilitate end-of-life discussions and support decision-making with families and caregivers
Provide hospice and palliative care services in patients homes or residences
Maintain accurate documentation in EMR systems
Represent Nurse America professionally in all patient, family, and physician interactions
Qualifications
Active Registered Nurse (RN) license State of Maryland
Minimum 3 years of acute care and/or hospice experience
CHPN certification preferred
Strong knowledge of end-of-life and hospice care principles
Excellent communication, organization, and customer service skills
Proficiency in Microsoft Office 365 and EMR systems
Experience with Netsmart/MyUnity or Homecare Homebase is a plus
Valid driver s license and reliable transportation for travel within Montgomery County
