As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes as prescribed by physicians, ensuring compliance with applicable laws and agency policies. • Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements, syncing timely per LHC policy.
• Ensure patients meet home health eligibility and medical necessity guidelines.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on‑call and weekend rotation as needed to meet patient needs.
• Adhere to and participate in the agency’s utilization management model.

• Education:
  
  Current and unrestricted RN licensure in state of practice.
• Experience:
  
  1+ years of Home Health experience preferred.
• Licenses &
  
  Certifications:
  • Current Driver’s License and vehicle insurance required.
  • Current CPR Certification or ability to complete within 90 days of hire.
