At Home Hospice, part of LHC Group, is seeking a Nurse Practitioner to deliver palliative and supportive care in the home hospice setting. You will consult with the Hospice Medical Director and primary physicians to meet patient needs and coordinate care with an interdisciplinary team.
You’ll perform face-to-face visits within time limits, educate families, document care plans, and maintain accurate notes while upholding compliance and compassionate care standards in Texas.
