At Home Hospice, part of LHC Group and United Health Group family, seeks a Hospice Nurse Practitioner to provide consultation in palliative and supportive care for hospice patients. You will work within the IDG, coordinating plans of care and ensuring symptom management in partnership with physicians.
Primary responsibilities include face-to-face visits, documenting patient progress, and collaborating with the Hospice Medical Director and primary physicians to optimize care.
