Important: Candidates without prior experience in MDS will not be considered.

The MDS Care Plan Coordinator leads the MDS department, assists in coordinating discharge planning and assessing short‑term residents. Responsibilities include maximizing reimbursement, ensuring Medicare compliance, managing short‑term managed care cases, gathering information, assessing needs, establishing goals, providing interventions, coordinating comprehensive assessments, writing care plans, encouraging resident participation, ensuring nursing staff follow plans, reviewing notes, monitoring compliance, revising plans, and conducting weekly care plan meetings per OBRA and state requirements.

They complete MDS with utmost accuracy and ensure the highest level of reimbursement for the facility while complying with CMS Mega Rule guidelines. Point‑Click Care experience is a plus.

• Current state RN license and good standing.
• Meet all applicable federal and state licensure requirements.
• Attention to detail, follow‑through skills, and ability to prioritize multiple tasks.
• Ability to instruct others.
• Knowledge of general, rehabilitative, and restorative nursing and medical practices, procedures, laws, regulations, and guidelines governing long‑term care.
