Role Summary
The CHF Nurse Navigator demonstrates critical thinking and uses the nursing process to assess and meet the needs of heart failure patients by providing care coordination through the heart continuum. This position acts as a liaison between patients, families and caregivers and the healthcare delivery system to optimize patient outcomes while reducing barriers to care.
Responsibilities- Coordinates Multidisciplinary Program.
- Assists program assistant as needed:
Obtains and reviews reports from cardiology for multidisciplinary conferences. - Assists program assistant as needed:
Coordinates and distributes case summaries to multidisciplinary physicians to facilitate case discussion. - Assists in the identification of candidates for heart failure follow up.
- Summarizes each case presented and sends note to referring provider.
- Coordinates patient admission process when appropriate.
- Coordinates appointments for care as applicable.
- Collaborates with community resources to support patients/families, including co‑pay assistance.
- Handles FMLA & Disability paperwork.
- Manages new prescriptions and refills.
- Provides patient education on new regimes and heart failure care teaching.
- Schedules separate appointments with cardiology to facilitate timely treatment recommendations.
- Identifies potential and realized barriers to care and facilitates referrals to mitigate them.
- Reviews and prevents medically unnecessary patient admissions through assessment reviews and prior authorization issues.
- Communicates with patients and family 24-48 hours after initial visit to clarify concerns.
- Applies basic knowledge of insurance processes and their impact on care decisions.
- Provides psychosocial support and facilitates referrals during high emotional stress.
- Builds therapeutic and trusting relationships with patients, families, and caregivers through effective communication and listening skills.
- Facilitates communication among multidisciplinary team members to prevent fragmented or delayed care.
- Participates in development of standards and monitors clinical quality indicators.
- Develops patient tracking database and action plans for indicators falling below benchmarks.
- Engages in community outreach activities and partnerships with community–based organizations.
- Participates in hospital initiatives, community, employee and physician education programs.
- Communicates effectively, timely and in an open, honest manner, sharing knowledge and skills with colleagues.
- Recognizes, respects, and trusts colleagues and their contributions.
- Identifies conflict, determines accountability for own role, and seeks resolution.
- Remains objective and empathetic.
- Uses critical thinking to obtain best outcomes.
- Performs miscellaneous duties per procedure and instructions.
- Bachelor’s Degree in Nursing.
- Master’s Degree in Nursing is preferred.
- Current license to practice as a Registered Nurse in North Carolina.
- BLS Certification. Recertification required every 2 years.
- 3 years’ experience in acute care as a Registered Nurse.
- Prior experience working directly with heart failure patients is preferred.
