Float LPN, Full-time
- Commitment Incentive up to $5k

• Provide safe, compassionate hospice nursing care under RN or physician supervision.
• Offer phone support to patients, families, and healthcare providers, assisting with immediate needs within LPN scope.
• Review supplemental orders, perform chart audits, and assist with office duties.
• Deliver end-of-life education to patients and families in compliance with regulations.
• Collaborate with the interdisciplinary team to assess patient needs and coordinate care.
• Participate in performance improvement activities and train new staff.

• Licensed Practical Nurse with current NC license.
• At least 2 years of recent clinical experience; hospice or home health preferred.
• Knowledge of Medicare regulations, licensing laws, and accreditation standards.
• Proficient in computer skills and CPR certified.
• Reliable transportation and ability to travel within the service area.
• Physical ability to lift up to 50 pounds and perform assessments.
