Schedule:

Part Time | 25-30 hours per week

The Case Manager Assistant provides administrative and clinical support to the case management team, ensuring coordination of post-discharge services, payer communication, and documentation tracking to facilitate timely, effective patient transitions. This role also supports coordinating family trainings, follow-up appointments and post-discharge services.

• College degree in a healthcare related field; preferred
• Currently licensed LPN or Social Worker; preferred
• Minimum of 1 year of experience in hospital or post-acute healthcare setting; preferred
• Proficiency in Microsoft Word, Excel, keyboard typing, and use of general office equipment; required
