Inpatient RN Care Manager

As an Inpatient RN Care Manager at Mercy, you will coordinate patient care from admission through discharge, ensuring safe, high-quality, and cost-effective outcomes. You’ll conduct clinical assessments, lead discharge planning, and collaborate with physicians, nurses, and interdisciplinary teams to arrange post-acute services and community resources. Using your inpatient experience and EMR skills, you’ll monitor utilization, advocate for patients, support quality and readmission reduction efforts, and contribute to Mercy’s compassionate, innovative culture focused on continuous learning and professional growth.
