Case Manager (CM), Inpatient Rehabilitation

Location:

4313 S Pleasant Crossing Blvd., Rogers, Arkansas 72758

Schedule:

Full Time | Monday‑Friday | 8am – 5pm

Responsibilities
  • Lead interdisciplinary care coordination and discharge planning
  • Identify barriers to progress and devise solutions with the care team
  • Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines
  • Facilitate weekly team conferences and family meetings
  • Communicate patient progress and discharge plans to caregivers
  • Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
  • Support additional departmental needs as assigned
Qualifications and Requirements
  • Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT, or SLP
  • At least 2 years of case management or social work experience in an inpatient setting (rehab experience strongly preferred)
  • Strong communication, organization, and collaboration skills
  • Proficiency in Microsoft Office
  • Case Management or Rehab Nursing certification (CCM, ACM, RN‑BC, ARN) preferred
