Job Details
• Home health setting with a caseload of 4-5 patients per day, depending on territory and drive time,
• Day shift schedule, Monday through Friday, 8:00 AM - 4:30 PM,
• No weekend or on-call requirements,
• Mileage reimbursement provided at the IRS standard rate,
• Epic EMR used; OASIS documentation required,
Job Requirements
• Current, active OT license (license required if applicable),
• Minimum 1 year of home health experience required,
• Proficiency with Epic EMR required,
• OASIS experience required,
• Current BLS certification required,
• Valid driver's license and Social Security card required for consideration,
• Two professional references required for consideration,
Additional Information
• Provide occupational therapy services to patients in a home health setting across an assigned territory,
• Caseload and daily schedule may vary based on geographic territory and patient drive time,
