- Setting: Home Health
- Pay Range: $2519.60 - $2645.58
- Schedule: 07:00:00 to 15:00:00
- Contract Length: 13 weeks
- Candidate Type: Local and traveler friendly, Local candidates only - travel stipend not available, Traveler friendly - travel reimbursement available
- Deliver one-on-one, high-touch care to patients in their homes
- Conduct skilled nursing visits and perform assessments
- Create and manage individualized care plans
- Collaborate with families and interdisciplinary providers to improve patient outcomes
