As a Patient Care Coordinator

• Develop and maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on Company's services available in a market.
• Primarily conduct facility visits at the physician's request to assist the program clinical team in determining eligibility.
• Ensure effective communication and collaboration with program staff and other field sales resources via weekly meetings and actively participate in weekly program business development meetings.
• Assist program in processing physician orders as directed.
• As a member of the sales team, you will report to a Market Executive of Sales.

• RN/LPN/LVN license.
• Bachelor's of Science in Nursing.
• 3 years clinical experience.
• Strong knowledge of governmental regulations, Medicare eligibility requirements, and comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Excellent customer service, account development capabilities, organization, time management, problem‑solving, communication and selling skills.
• Travel within an assigned territory and to sales meetings as required.
