Center Well Home Health is seeking an LPN/LVN to provide one-to-one skilled nursing care in patients’ homes under RN supervision. You will implement care plans, educate families, and document visits and outcomes to support recovery and independence.
The role requires a current nursing license, valid driver’s license with reliable transportation, CPR certification, and at least two years of LPN/LVN experience in a clinical setting. 40 hours per week with pay range $53,000–$73,000 annually.
