Enhance Rehabilitation is currently seeking qualified Physical Therapist candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist
, you will be part of a growing team providing the highest quality care for patients in a SNF setting
. In this role, you will evaluate and treat patients following an individualized plan of care.

Responsibilities:

Providing excellent care for our patients

Providing excellent customer service to our patients and family members

Collaborating with facility staff to ensure excellent patient care

Evaluating new patients, developing plans of care, and setting measurable, functional goals

Treating patients, addressing their concerns, and educating them on their injury, progress, and probable outcomes

Supervising Physical Therapist Assistants

Re-evaluating patient progress, revising plans of care, and resetting goals

Completing timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes
