Job Summary

The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with an emphasis on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Essential Job Functions / Responsibilities
  • Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of the nursing process.
  • Assesses and evaluates patient’s status by:
    • Writing and initiating plan of care.
    • Regularly re‑evaluating patient and family/caregiver needs.
    • Participating in revising the plan of care as necessary.
    • Initiating the plan of care and making necessary revisions as patient status and needs change.
    • Using health assessment data to determine nursing diagnosis.
    • Developing a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including the patient and the family in the planning process.
    • Initiating appropriate preventive and rehabilitative nursing procedures and administering medications and treatments as prescribed by the physician.
    • Counseling the patient and family in meeting nursing and related needs.
    • Providing health‑care instructions to the patient as appropriate per assessment and plan.
    • Assisting the patient with activities of daily living and facilitating self‑sufficiency and optional comfort care.
    • Acting as case manager when assigned by the Clinical Supervisor and coordinating patient care for an assigned caseload.
  • Communication:
    • Completing, maintaining and submitting accurate and relevant clinical notes regarding patient’s condition and care given.
    • Recording pain/symptom management changes and outcomes as appropriate.
    • Communicating with the physician regarding the patient’s needs and reporting changes in condition; obtaining/receiving physicians’ orders as required.
    • Communicating with community health‑related persons to coordinate the care plan.
    • Teaching the patient and family/caregiver self‑care techniques and providing medication, diet and other instructions as ordered.
    • Working in concert with the interdisciplinary group and maintaining a safe environment for the patient.
    • Assisting the patient, family/caregiver and other team members in providing continuity of care.
    • Cooperating with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
    • Attending interdisciplinary group meetings.
  • Delegated responsibilities for nursing services: supervising and teaching other nursing personnel, including licensed practical nurses and nurse aides, and evaluating the effectiveness of delegated acts performed under the registered nurse’s supervision.
  • Additional duties:
    • Participating in on‑call duties as defined by the on‑call policy.
    • Ensuring that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required, and maintains professional knowledge through continuing education and participation in in‑service classes.
  • Actively participating in quality assessment performance improvement teams and activities.
Qualifications
  • Graduate of an accredited school of nursing.
  • One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
  • Current licensure in state and CPR certification.
  • Bachelor’s degree, with one (1) year of hospice care experience preferred.
  • Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills; nursing skills per the competency checklist.
  • Physical ability to walk or stand for prolonged periods, lift, position, transfer patients, lift supplies and equipment, reach, stoop, bend, kneel, crouch, and possess adequate visual acuity and hearing to perform required nursing skills.
  • Licensed driver with an insured automobile in good working order as required by state or organization.
