Job Description:

* Communicates with healthcare providers to ensure seamless care transitions. Educates patients on their diagnosis, treatment options, and what to expect during treatment.

* Assists patients in accessing resources such as financial assistance, transportation, and support services.

* Tracks patient progress and adherence to treatment plans.

* Facilitates communication between patients and their healthcare team.

* Organizes and participates in patient education and support group activities.

* Advocates for patient needs within the healthcare system.

* Monitors and evaluates patient outcomes to improve care processes. Performs other duties as assigned.

* Coordinates appointments and follow-ups to ensure timely access to care.

* Provides emotional support and medical information to patients and their families. Guides patients through the entire cancer care continuum, from diagnosis to survivorship.

Knowledge, Skills, and Abilities:

* Computers, telephone, fax machine, copier. [Required]

* Oncology Nursing Skills [Required]

* Knowledge of hospital and community resources [Required]

* Ability to coordinate services for patients in a timely way [Required]

* Oncology Nursing [Preferred]

Education:

* Associate's of Nursing [Required]

* Bachelor's of Nursing [Preferred]

Work Experience:

* 1+ years acute care nursing experience [Required]

* 2+ years position related experience [Preferred]

Licenses and

Certifications:

* Registered Nurse (RN) [Required]

* Basic Life Support - CPR Cert (BLS) [Required]

* Oncology Certified Nurse (OCN) [Preferred]

Pay Range:

$31.44 - $54.92
